Evidence Portfolio

Hi everyone,

We have added some Evidence Portfolio examples into the Current Intern and Preceptors area of the website.

We have also had a few queries about what needs to be sent in this week with Assignment 2 and Appraisal 1.

You need to send in:
- 2 x Achievement Goal Sheets (1 for February and 1 for March)
- 4 x Primary Healthcare Evidence Forms (2 for February and 2 for March)
- 4 x Prescription Intervention Forms (2 for February and 2 for March)
- 4 x Inpatient Clinical Intervention Forms (2 for February and 2 for March) Hospital Interns Only
- 1 x Evidence Record for Pharmaceutical Products Form. (we understand that there is not much extemporaneous compounding in some pharmacies so you only need to fill in as much as you can for February and March).
- Monthly Diary (February, March)


The:
- Intern CPD Record Sheets (1 per month)
- Record of Professional Development Sheet
- Other Placement Experience Sheet
- Final Reflection Summary,
Do not need to be submitted until the end of the year, but you will need to be working on them throughout the year.
